Advanced Directive for Healthcare - $50.00 flat fee
This is a legal document where you lay out all of your specific medical requests, such as your anatomical gifts, whether you want life sustaining treatment, and whether you have specific religious or personal limitations on what doctors can and cannot do for you, such as receiving donor blood. This document is particularly important to give to your doctors before you have extreme medical needs so that they know exactly what to do if or when something catastrophic happens to you.
Healthcare Power of Attorney - $50.00 flat fee
This is a legal document that gives a trusted person the ability to make healthcare decisions on your behalf if you become incapacitated and unable to make decisions for yourself. This can include giving consent to medical care that you need, selecting healthcare providers and facilities, and/or signing do-not-resuscitate consent forms on your behalf.
General Power of Attorney - $50.00 flat fee
This is a legal document that allows other people to make important decisions on your behalf, such as dealing with bank accounts, mortgages, or utility providers. A power of attorney is important because you may find yourself incapacitated and no longer able to make decisions for yourself, and this document provides your trusted friend or family member with the ability to immediately step in and make decisions while you cannot. Without that power of attorney, your trusted friend or family member would need to go to court to have a guardian appointed on your behalf, which can be costly and also a lengthy process that would delay important decisions.
